2023 Flu Vaccination Clinics, Provided by Barton County Health Department

Available to all USD 428 Staff and Students (with parental/guardian consent), see schedule below.

Outreach Clinic Reminders:

• We are only providing flu vaccinations this year.

• Forms will need to be completed and returned to us prior to outreach clinics to designated populations. This will allow time to review for completion. If not returned by date due, outreach will be rescheduled if there are openings. **Additionally, insurance information MUST BE included for all staff and students. Insurances will be filed if applicable – including staff.

• No verbal consent from parent/guardian will be taken at outreaches.  If written consent is not available, then immunizations cannot be administered.
