Watching Young Artists Grow
Parents of incoming Preschoolers and Kindergartners to USD 428 are asked to complete the Ages & Stages Questionnaire (ASQ). ASQ is a child screening/monitoring program. The questionnaires found at the links below are screening tools that will provide a quick check of your child’s development and ensure our kindergarten classrooms are ready to meet the needs and learning goals of our students this Fall. The information you supply will help reveal your child’s strengths, uncover any areas of concern, and determine if there are resources or services that may be useful for your child or family.
We would like you to complete the questionnaire by clicking on the link(s) for the building your child will be attending. You will be directed to a welcome page for your building, and then the link to the survey. Please try the activities on the questionnaire with your child and record what you see and any concerns you’d like to share.
The purpose of collecting this information is two-fold.
The Kansas State Department of Education is requiring the collection of Preschool & Kindergarten Readiness as part of the accreditation process in order to have data readily available to discuss how prepared students are coming to school and to assist in funding determinations for Pre-K programs.
In addition to the KSDE requirement, USD 428 will also be using this information to ensure we have programs in place to meet the needs of all students coming into our schools.
USD 428 preschool students must complete the screening tool again before entering Kindergarten. The assessment is specific to the child based on their age in months so results from previous completions will be different.
