School Vaccination Clinics

Influenza Vaccinations are available to all USD 428 Staff and Students (with parental/guardian consent); see schedule below.

Outreach Clinic Reminders:

• BCHD will provide flu vaccinations only this year.
• Please complete and return forms to your school nurse one week before the outreach clinic at your school. This will allow time to review for completion. **Additionally, insurance information MUST BE included for all staff and students. BCHD will file insurance information, if applicable, including for staff.
• No verbal consent from a parent/guardian will be taken at outreach clinics.  If written consent is not available, then immunizations cannot be administered.
